Instructions for inclusion in study

Thank you for your interest in participating in a research project “Impact of First Aid Farm Quest CD on Emergency Preparedness and Risk Reduction". This webpage describes the procedure to participate in the evaluation of the use of CD-ROM technology to teach youth (5th and 6th graders) important principles of first aid and safety. The instructions refer to "teacher". A teacher may be a classroom teacher, an organizational leader or a parent.

As an incentive, we will offer each participating teacher a $50 gift card from several vendors (your choice) which you will receive once the posttest adminstration is complete.  

Procedure for obtaining CD:

1) To request a free copy of the CD “First Aid Farm Quest”, complete and submit the request form.  The CD,    with a Teacher’s Guide, will be mailed to you.

2) We will provide all the necessary materials - The CD that can be installed to as many computers desired; a Teacher's Guide with supplemental activities (that can be done for those students who finish early); a brief teacher questionnaire, and a pre-paid reply envelope for returning the parental consent forms to us.

The Teacher's Role in the Study:

 1)  Download the parental consent form, copy the needed number of forms and have parents complete.  Once all parent consent forms are collected, return them in the envelope provided to the teacher. In the event a parent does not consent to have his/her child participate in the study, the child still may still complete the pre-test and post-test and work through the CD. Only data collected from youth who assent and parents who consent will be used for analysis.

 2) Assign student(s) to take pre-test. To log into pre-test, the teacher will type in the password gandor. Each student will complete an online pre-test and press the submit button. No more than 15 minutes is needed for pre-test adminstration.

 3) Install CD on Server or C drive. This program was developed to allow the user to exit the program and resume work at a later time. There are two choices for installation. Server installation allows a user to return to any computer to complete the CD while a C drive installation will require the user to return to the same computer.

Server Installation Instructions:

A) Quit the Auto install. Explore the CD and copy the CD contents (all files and folders) to your server.

B) Run the Setup application from the server.

C) Users must have read and write privileges to the sub folder called "data" so that users can quit the program and return to where they left off at a later time.

C drive Installation Instructions:

A) Insert CD into CD drive.

B) A pop-up window will appear with an option to Install or Quit. Click on "Install".

C) After install is complete, go to Start, Programs and choose "Farm Quest".

4) Assign each student to complete the First Aid Farm Quest CD-ROM. The average time to complete all modules is 1 hour. A printable certificate of completion is available once the student works through all content.

 5) Each student is asked to complete the online post-test within 2 weeks of viewing the CD. The password for the post-test is gandor.

6) Once post-test administration is complete, a gift card for $50.00 will be sent to the teacher.
